Abstract:
Atrial fibrillation (AF) is a leading cardiac arrhythmia strongly linked with the risk of ischemic stroke. Amiodarone, a class III antiarrhythmic medication, is widely utilized for rhythm control in AF. However, emerging large-scale studies and clinical data reveal a concerning association between oral amiodarone therapy and increased stroke incidence in certain patient populations. This article reviews the epidemiology, pathophysiology, clinical data, and risk assessment related to ischemic stroke following oral amiodarone therapy in AF patients, and suggests evidence-based management to reduce cerebrovascular risk.

INTRODUCTION:
Atrial fibrillation significantly elevates the risk of thromboembolic stroke due to disorganized atrial contraction and subsequent blood stasis, potentially resulting in left atrial thrombus formation. The use of antiarrhythmic agents like amiodarone aims to restore sinus rhythm and reduce morbidity. Nevertheless, the safety profile of amiodarone concerning cerebrovascular outcomes warrants critical examination—as multiple recent cohort studies indicate a possibly heightened risk for ischemic stroke in specific patient subgroups.
Objectives
· Evaluate the incidence and risk factors for ischemic stroke in AF patients treated with oral amiodarone.
· Summarize mechanistic insights and clinical trial data regarding this association.
· Provide recommendations for clinicians regarding stroke prevention in this context.
MECHANISMS LINKING AMIODARONE AND ISCHEMIC STROKE:

DISCUSSION:
Although amiodarone remains a mainstay for rhythm control in AF due to superior efficacy in maintaining sinus rhythm, its association with increased ischemic stroke risk mandates a cautious, protocol-driven approach. Particular vigilance is needed in patients with low baseline stroke risk and those with concomitant digoxin therapy. Ensuring uninterrupted, effective anticoagulation and careful monitoring of drug interactions are critical for minimizing thromboembolic complications.
CONCLUSION:
Oral amiodarone therapy in patients with atrial fibrillation is independently associated with an increased risk for ischemic stroke, especially in the absence of adequate anticoagulation and in combination with digoxin. Close clinical monitoring, thorough risk stratification, and education are paramount to prevent catastrophic cerebrovascular outcomes. Integrating evidence-based risk mitigation strategies can help clinicians safely utilize amiodarone while minimizing the risk of preventable stroke.
